Most runners know that flat feet can cause pain. But here is something far fewer have considered — even when there is no pain at all, flat feet may already be quietly undermining the efficiency of every run you do.
Flat Feet Are About Far More Than How Your Foot Looks
When most people think about flat feet, the instinct is to think in simple terms: "My arch sits lower than average" or "My feet ache after a long day on them."
But for runners, the implications of flat feet go considerably further than that. Flat feet have a direct bearing on your propulsive efficiency with every stride, on how impact forces are distributed across your joints, and on your long-term injury risk — whether you feel it right now or not.
To understand why, we need to start with one foundational concept: overpronation (Overpronation).
What Is Overpronation — and What Does It Actually Do to Your Running?
In a normal running gait, the foot moves through a natural and purposeful mechanical cycle:
Pronation (Pronation) → Supination (Supination)
During the early phase of each stride, the foot pronates — rolling inward in a controlled way — to absorb the impact of landing. Then, as the stride progresses toward toe-off, the foot transitions into supination: the arch rises, the foot structure locks into a more rigid and stable configuration, and all of that stored energy is converted into the propulsive force that drives you forward into the next step.
When the arch collapses, however, this transition breaks down. The foot remains in a softer, less stable pronated state throughout more of the gait cycle — unable to complete the shift from pronation to supination with the timing and degree that efficient running demands.
The result?
At the precise moment your foot needs to be rigid enough to transmit force into the ground — it isn't.
It is the mechanical equivalent of trying to sprint off a sponge. The force is there. The intention is there. But the structure beneath you absorbs what should be propelling you forward — and efficiency drops with every single step.
The Chain Reaction: What Overpronation Does to the Rest of Your Body
This is not an isolated mechanical problem. It sets off a chain of downstream consequences that affect every structure from your foot to your hip.
❶ Loss of Propulsive Efficiency
Without sufficient stiffness at toe-off, every stride is leaking energy that should be driving you forward. The effort goes in — but the return doesn't come back. Over time, this makes running feel harder, and both speed and endurance suffer as a result.
❷ Altered Load Distribution Across the Lower Limb
The mechanical changes at the foot don't stay at the foot. They travel upward, shifting the alignment of the ankle, knee and hip — meaning the entire lower limb is absorbing impact at angles it was never designed to sustain.
❸ Compensatory Overload at the Knee, Ankle and Calf
When the foot cannot do its job, the joints and muscle groups around it step in to compensate. This is not a sustainable arrangement. The cumulative effect is accelerated fatigue — and, over time, overuse injuries that seem to come out of nowhere.
❹ A Long-Term Injury Risk That Builds Quietly in the Background
Even if everything feels manageable today, continuing to run through a compromised mechanical pattern is not a question of whether injury will come — it is a question of when.
No Pain. Does That Mean No Problem?
This is one of the most common — and most costly — assumptions we encounter in the clinic.
Flat feet don't always cause pain directly. But when any of the following are also present, the impact on both running performance and injury risk becomes substantially more pronounced:
🔴 Intrinsic muscle weakness — insufficient strength in the small muscles of the foot, leaving the arch without the active support it needs
🔴 Restricted ankle mobility — limited range of motion at the ankle joint, forcing the body into more pronounced compensatory patterns throughout the gait cycle
🔴 Inadequate hip control — weakness in the gluteal muscles, making knee valgus (inward collapse of the knee) significantly more likely under load
When these three factors overlap, they are very often the true explanation behind a runner who has plateaued in performance, or whose injuries keep returning despite rest and treatment.
How to Address It — Systematically
The good news is that running performance problems rooted in flat feet and overpronation are genuinely addressable — through proper assessment and targeted, progressive training.
